Life after a major injury or diagnosis is radically different. Individuals often have to relearn basic tasks, and getting out of the home seems impossible. We empower our participants to re-engage in the community, increase their confidence and improve their quality of life.

 

Our Mission: Building confidence and independence for individuals with physical disabilities and neurologic conditions through physical therapy based skills training and adventure challenges.

Destination Rehab was founded as a 501(c)3 nonprofit in the spring of 2016 in Bend, Oregon. We provide Physical Therapist-led outings and training to promote improved community integration and quality of life, and are experienced working with people with physical disabilities and neurologic conditions. Our program offers group outings and one-on-one sessions, allowing us to provide a comprehensive approach to improving community engagement, independence and quality of life.

What We Do

We provide community outings and outdoor adventures for adults with physical disabilities and chronic conditions such as: brain injury, stroke, Parkinson's, multiple sclerosis, spinal cord injury and more. Physical therapy based activities, taking rehab out of the clinic and into the real world! We do this through 3 programs: group outings, individualized Physical Therapy sessions, and Adventure Rehab Camps.

Target 1: Increase number of participants across our program offerings.

Target 2: Develop and implement a sustainable funding strategy.

Target 3: Address and evaluate programming for key impact areas:
quality of life, community participation, self-efficacy, and physical activity.

 

Our Founder

Carol-Ann is passionate about combining her love of the outdoors with bringing people to a new level of ability in their daily function. Carol-Ann grew up in Eugene, then attended Willamette University before moving to attend Duke University’s Doctor of Physical Therapy program. She demonstrated leadership and service as Class President of her DPT class. Carol-Ann has experience working with patients with a variety of complex neurologic conditions. She is a licensed physical therapist in Oregon and a member of the Oregon PT Association, American PT Association and the Neurologic Section.
